Peninsula Baseball and Softball Association
I hereby consent to the Peninsula Baseball and
Softball association collecting, retaining, using, and disclosing images,
audio, photographs and videos of the registering Player, as well as biographical
information such as name, birthdate, school grade, height and weight, in print
and online publications and websites for purposes related to the PBSA programs
and activities, including, but not limited to, recognizing the Player's
achievements and promoting PBSA.
